You really failed in describing what the problem is, so only way of replying here is to ask more questions:
1) what OS do you have? 2) have you installed all the required drivers? 3) what client are you using? 4) are you running GL or software? 5) what kind of gfx settings do you have in your client?

Maybe some settings in this post about optimal video settings for nvidia cards and qw can help. http://www.quakeworld.nu/forum/viewtopic.php?id=3001

um... have you tried "universal solution" for quake-engine games? "Disable thread optimization" in nvidia control panel if i remember correctly.

The "universal solution" actually decreases your fps on multi-core systems. For example 3GHz core2duo with GTX 260 gives 758fps threaded optimization on and 734fps threaded optimization off. The difference is 3,1% but with lesser card like 8800GT the difference was even more pronounced, up to 10% difference favoring threaded optimization turned on (gigabyte turboforce 8800gt in this particular case).

jumps?
btw for play in software you only need to download ezquake installer and click the option while installing, this will make 2 client exes, ezquake-gl.exe (gl quake) and ezquake.exe (software) "the quieter you become, the more you are able to hear"
