Just looking at the archive it contains shambler.mdl, so my guess is it *is* Quake model format. Of course you won't get all the details without the texture, normal map and gloss map in there as well.

The header suggests that it is a PSK model. The extension says nothing, especially when engines often suggest simply renaming any models files to .mdl so they find them.
